About the Role

Neighborhood Association is seeking a Senior Mental Health Case Manager to join our team. As a key member of our staff, you will provide person-centered, strengths-based services to clients with mental health needs. Your primary objective will be to help individuals live fulfilling lives in their communities.

Key Responsibilities
  • Conduct assessments to identify client needs and recommend treatment modalities
  • Make referrals for substance abuse and addiction services as needed
  • Orient clients to our shelter program, requirements, and consequences
  • Develop and implement Independent Living Plans (ILPs) in collaboration with other social service providers
  • Assist non-working clients in obtaining public assistance
  • Track and monitor client progress, including completing bio-psycho-social assessments within 30 days of intake
  • Empower clients to become involved in their own planning and goal setting
  • Provide ongoing support and counseling to help clients achieve long-term self-sufficiency
Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, or a related field
  • Certification as an Alcohol and Substance Abuse Counselor (CASAC)
  • Strong interpersonal skills and ability to maintain confidentiality
  • Knowledge of community resources and ability to solve problems under pressure

We offer a competitive salary of $60,000 - $70,000 per year, depending on experience, along with opportunities for professional growth and development.
