Job Title: Licensed Practical Nurse Pediatric Home Care

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Community Care, Inc. as a Pediatric Home Health Nurse. In this role, you will provide high-quality, patient-centered care to children in the comfort of their own homes.

Job Summary:

The LPN will work under the guidance of a Registered Nurse (RN) to provide direct nursing care to patients in the Washington area. The ideal candidate will have a strong background in pediatric nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  1. Administer medications and perform GTube care as ordered by the physician.
  2. Provide one-on-one in-home care to a 13-year-old boy, including assistance with daily living activities (ADLs).
  3. Document patient progress and communicate effectively with the RN and other healthcare team members.
  4. Inventory and order supplies as needed.
  5. Participate in ongoing education and training to maintain current knowledge and skills.
Requirements:
  1. Graduate of a state-approved LPN program with a strong background in pediatric nursing.
  2. Current license to practice as an LPN in the State of Pennsylvania.
  3. Minimum of six months of experience in pediatric home health nursing.
  4. Current CPR certification.
  5. Reliable transportation and a valid driver's license.
