Job Title: Licensed Practical Nurse

UPMC Home Health is seeking a full-time Licensed Practical Nurse (LPN) to join our team in the Mount Washington, Greentree, Crafton, Corliss, Carnegie, and surrounding areas.

Job Summary:

We are looking for a skilled and compassionate LPN to provide high-quality patient care in a home health setting. As a Licensed Practical Nurse, you will work collaboratively with our healthcare team to deliver patient-centered care, promote health education, and support patients and families in achieving their health goals.

Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with healthcare team members to coordinate patient care, including referrals to specialists and other healthcare services.
  • Provide education and support to patients and families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduation from a Licensed Practical Nursing program approved by the State Board of Nursing.
  • Current licensure as a Licensed Practical Nurse in the state where the facility is located.
  • Experience in healthcare, preferably in a home health or hospice setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
  • Ability to lift, push, and pull patients and equipment, as needed.
