UPMC Home Health is seeking a full-time Licensed Practical Nurse (LPN) to join our team in New Kensington and surrounding areas. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Responsibilities:
  • Provide comprehensive nursing care to patients in a variety of settings, including home health and community-based care.
  • Collaborate with healthcare team members to develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and professional development to stay current with best practices and advancements in nursing care.
  • Contribute to a positive and supportive work environment by demonstrating a commitment to teamwork, empathy, and excellent communication skills.
Requirements:
  • Graduation from an accredited Licensed Practical Nursing program.
  • Current licensure as an LPN in the state where the facility is located or eligibility for licensure in a participating state under the multistate Nursing Licensure Compact.
  • Basic Life Support (BLS) certification.
  • Automotive insurance.
  • Act 33 and Act 34 clearances.
  • OAPSA clearance.
