We are seeking a dedicated and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in the comfort of their own homes.
Key Responsibilities- Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC)
- Assist patients with maintaining good personal hygiene, including bathing, grooming, skin care, and oral care
- Communicate basic principles of nutrition and observe and record food and fluid intake when necessary
- Assist patients with mobility, exercises, positioning, transfers, and ambulation per Plan of Care and CenterWell Home Health policy
- Document interactions with patients, caregivers, doctors, and other staff members appropriately
- Practice acceptable infection control principles
- Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency
- At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
