Job Description Summary

A professional member of the VNA team, responsible for delivering high-quality care to patients in their homes. The LPN is accountable for making decisions and providing care according to established regulations and policies.

Key Responsibilities

1. Patient Care

• Perform all skills, treatments, and procedures competently and within established regulations and licensing laws.

• Coordinate and communicate with other clinical team members any changes in the patient's medical or physical condition.

• Complete required documentation to communicate changes in the plan of care, or changes in medications, treatments, or visit frequency.

• Perform comprehensive physical and psychosocial re-assessment at each visit.

• Assess pain every visit using an appropriate pain scale tool based on the patient's abilities and complete documentation as required.

• Evaluate the outcome of pain and symptom control measures, create a plan for management of symptoms in collaboration with the RN, teach the patient and family as needed, and document all necessary information.

• Demonstrate knowledge of Visiting Nurse Association's policies and procedures for administering and recording medications.

• Adhere to the Fall Risk Assessment Process and complete documentation as required.

• Individualize the Pathway, Tasks, and Directives to guide skilled care for scheduled visits.

• In collaboration with the RN case manager, determine the appropriate visit schedule based on the patient's acuity.

• Communicate at a minimum of weekly with the supervising RN to discuss the patient's status and revisions to the plan of care as established by the RN with input from the LPN.

2. Medical and Nursing Plan of Care

• Demonstrate the skills and judgment necessary to implement the medical and nursing Plan of Care, nursing interventions, and procedures as necessary for the care of the patient.

• Demonstrate knowledge of commonly used medications, their action, dose, and side effects, and readily utilize resources for research.

• Observe the rights for medication administration: right patient, right drug, right dose, right method, right time, and right documentation.

• Demonstrate proper procedures in all methods of medication administration.

• Assess and document IV site, patency, and fluid administration, when applicable.

• Maintain accurate medication profiles and review medication profiles with the patient/family/caregiver at each home visit.

• Complete DRR and medication reconciliation upon admission and with every new medication.

• Demonstrate knowledge of commonly used medications' action, dose, and side effects, and readily utilize resources for research.

3. Routine Nursing Responsibilities

• Perform routine nursing responsibilities and other general tasks required to provide for the safety, emotional, spiritual, and physical comfort of the patient(s), based on established care plans.

• Collaborate with all members of the interdisciplinary team to meet the needs of the patient.

• Communicate with other clinical team members any changes in the patient's medical or physical condition resulting in a change in the plan of care.

• Participate in and initiate patient care conferences as needed.

• Involve and document patient/family/caregiver participation in the Plan of Care.

4. Reassessing Patient Needs

• Reassess the identified problems, care provided, and patient responses in order to meet patient and family needs to assure overall quality of care.

• Assist in the assessment of patient needs, then plan, implement, and document nursing care under the direction of an RN.

• Based on reassessment, the Plan of Care will remain the same or modified to meet the patient/caregiver's identified needs and notification of any changes will be communicated to the nurse manager or designee.

• Assess the patient/caregiver's response to the teaching plan in order to evaluate the effectiveness of the plan.

• Observe the patient's condition and report changes of condition and the need for nursing measures beyond their knowledge to the nurse manager or designee.

• Assess and address comfort/pain level each visit utilizing an appropriate pain tool based on the patient's ability.

5. Maintaining Accurate Documentation

• Maintain up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team, in order to assure quality care.

• Each identified problem must be addressed and appropriately documented in the patient record.

6. Patient Management

• Make scheduled home visits to patients during the day or evening shift, as evidenced by documentation in the clinical record and nursing daysheet or daily log.

• The LPN demonstrates responsibility and accountability for the plan of care for each patient on caseload under the direction of a Registered Nurse.

• Schedule and prioritize nursing visits according to patient status.

• Maintain Agency standard of productivity as per team standard.

• Provide verbal/written update of Plan of Care to third-party payors as required.

• Order durable medical equipment (DME) and/or supplies as needed.

• Arrange and obtain lab results per MD order.

• Coordinate and communicate any changes in the plan of care to the appropriate members of the interdisciplinary team.
