We are seeking a skilled Care Manager RN to join our team at Physicians Regional - Pine Ridge.
**About the Role:**
* Coordinate the overall interdisciplinary plan of care for patients, from admission to discharge.
* Act as a liaison between patients/families and healthcare personnel to ensure necessary care is provided promptly and effectively.
* Rely on experience and judgment to plan and accomplish goals. Perform a variety of tasks. A certain degree of creativity and latitude is required.
* Typically report to a head of a department/unit.
**Responsibilities:**
* Develop and implement individualized care plans for patients.
* Collaborate with healthcare teams to ensure seamless care transitions.
* Communicate effectively with patients, families, and healthcare providers to address concerns and needs.
* Monitor patient progress and adjust care plans as necessary.
**Requirements:**
* Registered nurse with at least 2 years of clinical experience.
* Associates degree required, bachelor's degree preferred.
* Familiar with standard concepts, practices, and procedures within a particular field.
