Job Summary

The House Supervisor/Clinical Command Center Nurse is a critical role within CommonSpirit Health, responsible for providing and coordinating care to individuals and families using a holistic approach in our onsite clinical command center. This position requires a licensed nurse with a minimum of 5+ years of recent experience and knowledge of care transitions.

Key Responsibilities
  • Function as a lead and resource for all Clinical Command Center functions and float to the role needed to support patient care, throughput, and transitions.
  • Collaborate and communicate effectively virtually with physicians, nurses, and other healthcare team members in the provision of clinical care across the continuum.
  • Assess and reassess virtually the medical, physical, and psychological status of patients by reviewing patient records, obtaining a comprehensive problem-oriented health history, and monitoring and evaluating diagnostic studies as medically indicated.
  • Interpret available data to establish a primary plan of care and intervene as required, participating through an interdisciplinary team approach and through multi-dimensional patient care conferences, ongoing patient assessments, and re-evaluation of patient problems or needs to develop a plan of care.
  • Prioritize clinical information with critical thinking to escalate patient instability with care plan and medical regimen.
  • Respond to routine, urgent, and emergent patient problems appropriately by demonstrating effective decision-making and provide follow-up to ensure that patient problems are resolved.
  • Provide direct observation via virtual technology, proactive surveillance of adverse physiological trends, and assurance of appropriate prophylactic strategies, medical chart review, and appropriate reaction to appropriate alerts.
  • Prioritize and manage multiple electronic devices and software applications, including but not limited to EMR, Call Management Systems, Patient monitoring tools, Google Workspace, and online schedules.
  • Coach and mentor other clinical team members.
  • Virtually educate patients and families on disease processes and procedures.
  • Work to exceed national benchmarks for quality care delivery, cost containment, and length of stay indicators.
  • Maintain patient privacy and confidentiality.
  • Responsible for staying current in department policies and workflows, in-service programs, continuing education programs, and fulfill yearly requirements as stated by policy.
  • Participate in achieving nursing division goals and support a shared leadership structure.
  • Assure appropriate level of understanding, awareness, and compliance with all applicable Joint Commission, federal, state, and agency laws, regulations, guidelines, and professional standards.
  • Through a defined reporting process, report clinical, administrative, or technical problems that result in medical errors, unsafe care, or sub-optimal communication between appropriate healthcare professionals.
  • Demonstrate work practices consistent with CommonSpirit Health and department-specific safety, security, and infection control policies.
  • Collaborate with internal and external patients/clients within and across systems to ensure appropriate care transitions.
Qualifications
  • RN required.
  • Minimum of 5+ years of recent experience as a licensed nurse and knowledge of care transitions.
  • Knowledge and ability to facilitate patient care across the continuum.
  • Experience and knowledge in the use of Electronic Medical Records (EMR).
  • Bachelor's of Science in Nursing preferred.
  • Cardiac monitoring experience preferred.
  • Current license Registered Nurse.
  • Nurse Licensure Compact (NLC) or multi-state licensure.
  • Valid Basic Life Support (BLS) certification required.
  • Knowledge of Advanced Life Support (ALS) (preferred).
  • Ability to use multiple technologies; audio/visual.
  • Ability to build alliances in a highly-matrixed environment. Demonstration of consistent integrity and ability to meet commitments.
  • Expert skill in leading teams and building cooperation.
  • Communicating ideas and information clearly, concisely, and effectively.
  • Innovative, progressive, communicative, and responsive.
  • Problem identification/resolution skills.
  • Exceptional organizational skills required, with the ability to function in multiple roles.
  • Must remain flexible and comfortable with ambiguity within a very demanding, constantly changing environment.
  • Display sound judgment, tact, discretion, and ability to develop and sustain a cooperative working relationship with all levels of employees, leaders, physicians, and customers.
  • Knowledge of standards, procedures, and equipment relating to the monitoring and documenting of cardiac rhythms.
