About the Role

Sangre de Cristo Community Care is seeking a skilled and compassionate Registered Nurse to join our dynamic home health team in a part-time capacity. As a key member of our team, you will play a vital role in delivering exceptional, high-quality care to patients in Southern Colorado.

Responsibilities
  • Plan, coordinate, implement, and evaluate the nursing care of all patients.
  • Utilize clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs.
  • Work closely with the interdisciplinary team to provide a continuous, appropriate, comprehensive, and responsive plan of care for patients.
Requirements
  • Must be at least 21 years of age.
  • A valid non-restricted RN license in the state of Colorado is required.
  • Must have a valid non-restricted driver's license.
  • Flexibility and availability to work a weekend schedule if needed.
  • At least 1 year of home health or hospice experience preferred.
