Job Summary

Modivcare is seeking a compassionate and dedicated Certified Home Health Aide to provide professional, non-clinical personal care and caregiving services to enable clients to remain healthy and independent in their home and community settings.

Responsibilities
  • Assist with instrumental activities of daily living, such as bathing, grooming, dressing, and toileting.
  • Provide personal hygiene improvement and maintenance, including transferring and ambulation assistance.
  • Remind clients to take medication and maintain a clean, comfortable, and safe living environment.
  • Transport clients, as requested or approved, and prepare meals.
  • Perform housekeeping and laundry duties, and assist with grocery shopping.
  • Support clients in completing activities of daily living and contribute to their quality of life by influencing patient safety, good hygiene, healthy eating, medication adherence, and clean home environments.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care.
