Job Summary:

Under the direction of the Administrator and/or Director of Nursing, the Patient Care Coordinator is responsible for organizing and coordinating patient activities in the Post Acute Care unit at Southern Tennessee Regional Health System - Winchester.

Key Responsibilities:

  • Coordinate patient activities and provide support to patients and families
  • Work closely with the healthcare team to ensure seamless care delivery
  • Develop and implement patient care plans in collaboration with the interdisciplinary team

Requirements:

  • High school diploma or equivalent required
  • Work experience in coordinating patient activities preferred
  • Certified Nursing Assistant (CNA) certification for the State of Tennessee preferred
  • AHA Basic Life Support (BLS) certification within 30 days of hire
