Job Summary:
Under the direction of the Administrator and/or Director of Nursing, the Patient Care Coordinator is responsible for organizing and coordinating patient activities in the Post Acute Care unit at Southern Tennessee Regional Health System - Winchester.
Key Responsibilities:
- Coordinate patient activities and provide support to patients and families
- Work closely with the healthcare team to ensure seamless care delivery
- Develop and implement patient care plans in collaboration with the interdisciplinary team
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare team members
Requirements:
- High school diploma or equivalent required
- Work experience in coordinating patient activities preferred
- Certified Nursing Assistant (CNA) certification for the State of Tennessee preferred
- AHA Basic Life Support (BLS) certification within 30 days of hire
Work Environment:
The Patient Care Coordinator will work in a fast-paced healthcare environment, requiring strong critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
