About the Role

Public Partnerships LLC is seeking a highly motivated and results-driven Support Broker to join our team. As a Support Broker, you will play a critical role in empowering individuals with disabilities or chronic illnesses and aging adults to remain in their homes and communities.

Key Responsibilities
  • Provide information and education to Medicaid members who participate in self-direction programs, helping them navigate their employer responsibilities.
  • Evaluate participants' home environments, identifying and resolving safety concerns through our mandated escalations process.
  • Work with participants and their authorized representatives to ensure they have the necessary support to remain in their homes.
  • Develop and maintain strong relationships with participants, their families, and other stakeholders to ensure seamless support services.
Requirements
  • Bachelor's degree in social work, case management, or a related field.
  • At least one year of experience working with vulnerable populations in their homes.
  • Strong working knowledge of Microsoft Office products, including Outlook.
  • Familiarity with CRM tools and electronic medical records.
  • Valid driver's license and satisfactory driving record.
