Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Compassus team. As a Hospice LPN, you will play a vital role in delivering high-quality patient care and supporting our interdisciplinary team.

Key Responsibilities
  • Provide nursing services as directed by the Registered Nurse, including administering medications, treatments, and gathering clinical data.
  • Submit accurate and timely documentation, including patient visits and care plans.
  • Communicate effectively with RNCM, DCS, and physicians to ensure seamless care coordination.
  • Participate in quality performance improvement projects and clinical record reviews.
  • Provide education and reinforcement of teaching related to patient care, disease processes, symptom management, and medications.
Requirements
  • Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse license in the state(s) of employment.
  • Current CPR certification.
  • Minimum of one (1) year of full-time experience as an LPN.
  • Experience in skilled nursing, preferably in hospice, home health care, oncology, or emergency care.
