At US Tech Solutions, we are committed to delivering exceptional healthcare services to our clients. As a Nurse Case Manager, you will play a vital role in ensuring the well-being of our senior members.

Your responsibilities will include developing and implementing individualized care plans, facilitating member access to community resources, and participating in primary care team meetings. You will also collaborate with healthcare professionals to ensure smooth transitions of care and provide education and support to members and caregivers.

Requirements:

  • \
  • Bachelor's degree in Nursing and current Massachusetts RN license required\
  • Minimum 5 years of clinical nursing experience, with a focus on geriatric/chronic illness populations\
  • Proficiency in computer use, the Internet, and health information technology essential\
  • Case management certification a plus\

Compensation: $95,000 - $130,000 per year, commensurate with experience
