Lifeline Connections is a community-based behavioral health organization that provides confidential and compassionate care to individuals experiencing substance use and/or mental health conditions. Our unique model addresses short-term outpatient primary care, basic mental health, and addiction health services.

Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team as an Interim Healthcare Provider. The successful candidate will be responsible for providing primary care services, including chronic illness management, minor injury treatment, and medication management. The ideal candidate will have experience working with underserved populations and possess strong clinical skills.

Responsibilities
  • Provide interim care and medications (if indicated) and screening for chronic illnesses such as hypertension, diabetes, hyperlipidemia, hypothyroidism, etc.
  • Treat minor injuries such as strains, sprains, and non-complicating extremity fractures and dislocations within the physician's scope of practice.
  • Treat superficial minor lacerations, abrasions, minor first and second-degree burns within the provider's scope of practice.
  • Provide refills for medications for chronic medical issues and mental health (within comfort and consult with MH providers).
  • Evaluate, examine, and provide individualized treatment plans for acute and chronic issues, and minor injuries for patients.
  • Provide Hep C evaluation and treatment (can be trained for this).
  • Provide MAT evaluation, treatment plan, and medication management if needed (can be trained for this).
  • Order and interpret diagnostic tests including but not limited to labs and imaging.
  • Engage in collegial consultative relationships with other providers, including mental health providers, registered nurses, medical assistants, office staff, Residential Treatment Center staff, and Outpatient Treatment Center staff.
  • Counsel and instruct patients in health maintenance problems, including nutrition, diet, and weight reduction; risk factor prevention and modification; mental health referrals; self-care measures for health maintenance; family planning; STD prevention and treatment; and Hepatitis C management (can be trained for this).
  • Identify and appropriately refer patients to higher levels of care in a timely manner.
  • Assist the medical team in coordinating care that the patient receives from other providers and agencies and advocate for the patient within the healthcare system.
Requirements
  • Washington State Department of Health medical License required.
  • NP, preferred Board certified or Board Eligible in Family Practice or Internal Medicine required or equivalent training.
  • Board certified in Addiction Medicine preferred but not required.
  • CPR card.
  • Experience working with underserved, houseless, and patients with addiction preferred.
  • Experience with MAT medication management but not required.
  • Possess a current DEA credential with X waiver.
  • Ability to maintain client confidentiality.
  • Good verbal and written communication skills.
  • Great attendance.
  • Ability to work well with others.
Working Conditions

The incumbent performs duties in accordance with Washington Administrative Codes, Federal regulations governing confidentiality, and all of Lifeline Connections Policies and Procedures. The position requires that the incumbent maintain certification or qualification standards delineated in RCW 71.05, and RCW 71.34 and WAC 246-341 or its successors, and maintains licensure with the Department of Health.
