Primary Care Provider

We are seeking a skilled Primary Care Provider to join our Clinical team in Vancouver, Washington. As a Home Healthcare Specialist, you will conduct comprehensive assessments of Medicare, Medicaid, and Commercial Marketplace members/patients in their homes. Your role will involve performing detailed evaluations, identifying referrals, and making recommendations for follow-up care.
As a Comprehensive Health Clinician, you will be responsible for delivering high-quality healthcare services to elderly and chronically ill members. This is an independent contractor position with flexible scheduling options available.
The compensation for this role is $100 per completed visit. If you have 2 years of hands-on experience as a Nurse Practitioner and meet the required qualifications, we encourage you to apply.
- Required skills include knowledge of the HCC model, HEDIS, and STAR quality measures.
- You must be registered with Medicare and Medicaid, hold a current, unrestricted license in the state of assignment, and possess APRN certification.
- Experience in geriatrics, internal medicine, or primary care is preferred.
