Transform Patient Care with Encompass Health
As a Case Manager II at Encompass Health, you will play a vital role in delivering exceptional patient care and rehabilitation services. Our team is dedicated to providing a supportive and inclusive environment where you can thrive.
Key Responsibilities:
- Coordinate patient care from admission to post-discharge, ensuring seamless transitions and timely resource allocation.
- Develop and implement individualized treatment plans, collaborating with interdisciplinary teams to achieve optimal patient outcomes.
- Monitor patient experience, quality, and timeliness, making adjustments as needed to ensure exceptional care.
- Facilitate team conferences, coordinate treatment plan modifications, and complete required documentation.
- Stay up-to-date on regulations, company policies, and department operations, applying this knowledge to drive quality care.
Requirements:
- Must be qualified to independently complete assessments within the scope of practice of their discipline (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
- Hold an active license if licensure is required for their discipline within the state.
- Meet eligibility requirements for CCM or ACMTM certification upon entry into this position or within two years of entry.
- CCM or ACMTM certification required or must be obtained within two years of being placed in the Case Manager II position.
