Position Overview:
As a Family-Centered Treatment Specialist at Health Connect America, you will play a crucial role in delivering comprehensive support services to children, adolescents, and their families. This position emphasizes the importance of providing care within the family’s natural environment, ensuring that interventions are tailored to meet their unique needs.
Key Responsibilities:
- Service Delivery: Implement Family-Centered Treatment (FCT) services in accordance with established guidelines and model fidelity. Facilitate the progression through the four treatment phases, ensuring effective interventions.
- Certification and Training: Attain FCT certification within the first year of employment. Engage in ongoing training and skill development through internal programs.
- Clinical Documentation: Accurately complete and submit all necessary clinical records and FCT documentation in the Electronic Health Record (EHR) system, adhering to company protocols.
- Service Plan Development: Collaborate with the Treatment Team to create, maintain, and execute individualized Service Plans, ensuring regular updates and effective communication.
- Psychoeducation and Goal Setting: Provide educational resources as outlined in the Service Plan. Assist families in establishing weekly objectives and identifying actionable steps to achieve them.
- Referrals and Discharge Planning: Facilitate referrals to appropriate services and resources. Coordinate discharge plans, ensuring access to community resources for aftercare.
- Assessment and Coordination: Manage initial and ongoing assessment processes. Convene the Treatment Team and collaborate with both medical and non-medical providers.
- Behavioral Health Services: Coordinate and deliver behavioral health services in conjunction with other professionals and Treatment Team members, ensuring access to necessary evaluations.
- Quality Assurance: Participate in Quality Assurance and Quality Improvement initiatives, including audits and the development of improvement strategies.
