Job Summary

We are seeking an experienced RN to join our team as a Home Health Case Manager. This role involves providing direct care to patients in their homes, while also managing their caseload and coordinating with the interdisciplinary team to ensure optimal care.

Responsibilities
  • Provide direct care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Manage patient caseload, including coordination of care with the interdisciplinary team and monitoring of patient progress.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Conduct regular patient assessments to identify and address any changes in patient condition or needs.
  • Document patient information and care plans in a timely and accurate manner.
Requirements
  • Graduate of an accredited School of Nursing.
  • Current RN licensure in California.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
  • 2 years of recent relevant experience in home health or a related field.
Preferred Qualifications
  • Experience working with hospice patients and their families.
  • Knowledge of palliative care principles and practices.
  • Ability to communicate effectively with patients, families, and healthcare professionals.
