Occupational Therapist Home Health Per Diem

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.
Key Responsibilities- Assess patients' daily living and work-related skills and develop personalized therapeutic retraining programs.
- Administer and interpret diagnostic and prognostic tests to evaluate patients' level of function.
- Collaborate with physicians and other healthcare team members to develop and revise treatment plans.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function.
- Review and expand retraining programs to meet patients' behavioral goals and rehabilitation program objectives.
- Consult with other healthcare professionals to develop vocationally oriented plans and coordinate care.
- Report patient status and progress to physicians, rehabilitation staff, and family members.
- Participate in care coordination and discharge planning activities.
- Master's degree from an accredited Occupational Therapy Program.
- Minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
