Occupational Therapist Home Health Per Diem

CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients manage chronic conditions or recover from injury, illness, or surgery.
Key Responsibilities- Assess and develop individualized treatment plans to improve patients' daily living and work-related skills.
- Administer and interpret diagnostic tests to evaluate patients' level of function and develop effective rehabilitation programs.
- Collaborate with healthcare teams to develop and implement comprehensive care plans.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function.
- Confer with patients, families, and healthcare teams to ensure seamless care coordination and discharge planning.
- Design and implement community reintegration activities to support patients' physical and psychological recovery.
- Recommend and design adaptive equipment to enhance patients' independence and participation in rehabilitation programs.
- Train and supervise Occupational Therapy Assistants and Home Health Aides to ensure high-quality care.
- Master's degree from an accredited Occupational Therapy program.
- Minimum six months of occupational therapy experience, with Home Health experience preferred.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
