Position Overview:
Millennium Physician Group is actively seeking a dedicated and skilled ACO Case Manager to join our dynamic team. This full-time role operates Monday through Friday from 8 AM to 5 PM.
Key Responsibilities:
The ACO Case Manager plays a crucial role in providing direct services aimed at helping patients achieve optimal health outcomes. Responsibilities include:
- Delivering client-centered services that connect patients and their families with appropriate healthcare and community resources.
- Ensuring timely and coordinated access to necessary medical and support services.
- Conducting comprehensive assessments, planning, and advocacy for patients.
- Monitoring patient progress and maintaining accurate documentation across all program areas.
- Exhibiting a thorough understanding of patient growth and development, and effectively communicating with diverse age groups.
Qualifications:
To be successful in this role, candidates must possess:
- A valid and active Registered Nurse license in the State of Florida.
- A minimum of 3 years of clinical experience, with at least 1 year in case management preferred.
- Strong computer proficiency, particularly in Excel, and familiarity with EMR systems; knowledge of Athena is a plus.
