Position Overview:

The Case Manager will be responsible for managing cases related to child and adult abuse or neglect, ensuring that at-risk clients receive the necessary protective services and support.

Key Responsibilities:

  • Conduct timely, in-person assessments of reported cases of child maltreatment, addressing immediate safety concerns.
  • Develop and implement safety plans to mitigate identified risks.
  • Engage in diligent search efforts to locate families when contact cannot be established.
  • Utilize Solution Based Casework (SBC) methodologies to enhance service delivery.
  • Collaborate with families to create and utilize the Family Functioning Agreement and Action Plan to achieve desired outcomes.
  • Complete genograms to identify and leverage family support networks.
  • Work with community partners to build supportive systems for children and families.
  • Ensure adequate frequency and quality of visits with families to maintain safety and well-being.
  • Identify and address the physical, educational, and mental health needs of clients.
  • Establish and monitor permanency goals for children in care.
  • Ensure stable and appropriate foster care placements aligned with permanency goals.
  • Facilitate timely permanency for children in accordance with relevant regulations.
  • Maintain sibling connections in foster care placements unless separation is necessary.
  • Support and promote positive relationships between children and their biological families.
  • Document all case management activities in compliance with established policies.
  • Celebrate and acknowledge positive outcomes with families.
  • Utilize a personal vehicle for work-related travel, including transporting clients as needed.
  • Participate in after-hours on-call duties as required.
  • Engage in ongoing professional development and training opportunities.
Our Case Managers primarily work in an office setting, with the flexibility to complete documentation remotely as needed. The role involves working closely with children and families in the designated service areas.

Compensation: $33.50 per hour

Mileage reimbursement is provided for work-related travel.
