Job Summary: As a key member of the patient care delivery team, the Emergency Department Case Manager serves as a vital link between patients, families, and healthcare providers. This role requires clinical expertise, discretion, and independent judgment in determining medical necessity and appropriate patient care.

Key Responsibilities:

  • Utilize clinical knowledge and specialized emergency medicine case management methodologies to provide high-quality patient care
  • Collaborate with ED physicians, staff, and inpatient providers to ensure seamless patient transitions and optimal outcomes
  • Apply advanced communication skills to effectively engage patients, families, and healthcare teams in the care planning process
  • Maintain accurate and timely documentation of patient interactions, progress, and outcomes

Education and Experience: A Bachelor's Degree in Nursing is required; or an Associate's Degree in Nursing with six years of Case Management experience may be considered in lieu of a Bachelor's Degree. A minimum three years of bedside nursing experience in a hospital setting and one year of Case Management experience are preferred.

Knowledge and Skills:

  • In-depth knowledge of case management practice, methodologies, and principles
  • Substantial knowledge of basic nursing concepts, patient assessment, and care coordination
  • Strong analytical, problem-solving, and decision-making skills
  • Excellent communication, interpersonal, and leadership abilities
  • Able to work independently with minimal supervision and prioritize multiple tasks effectively

Licensures and Certifications: An active California Registered Nurse (RN) license is required; Basic Life Support (BLS) certification from the American Heart Association is also necessary.
