Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team in Nassau County. As a Registered Nurse, you will be responsible for providing high-quality patient care and contributing to the overall success of our healthcare team.

Responsibilities
  • Assess patients' physiologic health status and develop individualized care plans.
  • Conduct patient interviews, explain policies and procedures, and review patient charts to ensure accurate and timely care.
  • Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status, and adjust care plans as needed.
  • Determine mobility, sensory deficits, prostheses use, and skin condition, and adjust care plans accordingly.
  • Assess patients' pain levels and develop effective pain management plans.
  • Communicate and document patients' physiologic health status and care plans to ensure seamless care.
  • Assess patients' psychosocial health status, including perception of medical/nursing care and expectations.
  • Elicit patients' coping mechanisms, knowledge levels, and ability to comprehend, and identify cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction.
  • Communicate and document psychological status and care plans, including support group and counseling services.
  • Identify patient outcomes, develop criteria for measurement, and establish individualized patient goals.
  • Formulate care plans and establish priorities to achieve expected outcomes.
  • Coordinate the cost-effective use of supplies, equipment, and medication to achieve expected outcomes.
  • Document care plans and collaborate with physicians and other healthcare team members.
  • Implement care plans and maintain constant vigilance over patients' care to ensure safety precautions are followed.
  • Exercise professional skills related to care plans and reassess patients as needed.
  • Correctly administer prescribed treatments and use equipment necessary for patient care.
  • Provide emotional support to patients and significant others.
  • Apply scientific principles in performing procedures and carry them out safely, timely, and efficiently.
  • Make accurate observations of patients' conditions during treatments/procedures and report and record same as appropriate.
  • Keep accurate documentation of patients' treatment, activity, and condition, as well as patients' responses to medical and nursing interventions.
  • Use appropriate methods of documentation according to departmental policy.
  • Act rapidly and effectively during emergency situations, managing self, patients, and other employees.
  • Provide a calm, quiet, restful atmosphere and communicate effectively with patients' families or caretakers.
  • Participate in planning for discharge and coordinate referrals as appropriate.
  • Provide discharge instructions to patients and significant others.
  • Evaluate care provided for patient outcomes and demonstrate ability to measure effectiveness of care provided.
  • Perform variance analysis related to outcome data for performance improvement.
  • Design, implement, and evaluate systems to improve care in the unit.
  • Keep accurate documentation of patients' treatment, activity, and condition.
  • Use appropriate methods of documentation according to departmental policy.
  • Collaborate with other care team members in planning and carrying out treatment regimens and provide direction to other members of the care team.
  • Collaborate with physicians on patients' care plans.
  • Accurately interpret and implement treatment regimens as prescribed by physicians.
  • Assist physicians during treatments and/or diagnostic procedures.
  • Keep patient care managers/designees and/or physicians abreast of changes in patients' conditions and/or treatments as appropriate.
  • Use clinical judgment in delegating assignments to patient care associates, support care associates, and clerical service associates in providing patient care and ensure that assignments are completed in a timely fashion.
  • Perform grade I-IV decubitus care.
  • Perform preventive skin care measures.
  • Apply simple dressings, maintaining principles of aseptic technique.
  • Apply warm and cold compresses, consistent with facility procedure.
  • Perform irrigations, consistent with facility procedure.
  • Perform other procedures related to skin care as necessary.
  • Administer medications correctly and safely.
  • Correctly identify medication action, dosage, side effects, and implications for patients.
  • Meet standards on medication administration examinations.
  • Demonstrate preparation of local solutions.
  • Demonstrate preparation of I.V. drips.
  • Administer and document medications correctly.
  • Administer IVP medication as per department policy and procedure.
  • Educate patients and significant others related to drug and food interactions.
  • Participate in patient and family education.
  • Provide patients with explanations of their conditions as indicated.
  • Communicate assessment data in an orderly fashion by recording, updating, and verbalizing pertinent information to care team members and to appropriate agencies.
  • Recognize and utilize health teaching opportunities and resources/materials available for teaching.
  • Provide for early discharge planning and appropriate referrals for post-hospital care.
  • Evaluate the effectiveness of teaching by feedback from patients/families and document same.
  • Perform related duties as required.
Requirements
  • Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010, and obtain a BSN Degree within five (5) years of employment date.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • BCLS, preferred.
