We are seeking a skilled PRN/PT Social Worker MSW to provide patients and family members with education and advice to cope with emotional or physical problems. The ideal candidate will have experience in crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning, and psychosocial assessments.
Key Responsibilities- Conduct detailed psycho-social assessments to interpret social, environmental, and medical needs of patients.
- Develop, implement, and evaluate treatment plans using advanced practice methods.
- Provide patient and family education, serve as a professional resource, and collaborate with the healthcare team.
- Serve as a liaison with community resources to facilitate smooth transition and placement of patients.
- Participate in performance improvement initiatives, including data collection and analysis, and continuing education programs.
- Masters degree in Social Work (MSW) from an accredited institution.
- 1 year of experience in social work, preferably in a healthcare setting.
- Lic Clinical Social Worker (LCSW), Lic Masters Social Worker (LMSW), or Lic Master Social Wrk AdvPrac (LMSW-AP) certification through the state of TX.
