Key Responsibilities
  • Collaborate with our RN Case Managers and Utilization Management team to develop and implement comprehensive care plans for patients.
  • Identify and address psycho-social barriers to discharge, ensuring a smooth transition for patients and their families.
  • Provide education and support to patients and their caregivers, empowering them to take an active role in their care.
  • Participate in multidisciplinary teams to develop and implement strategies to improve patient outcomes and satisfaction.
Requirements
  • Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school.
  • Licensed CSW or MSW as required by the state of Delaware.
  • Acute Care Social Work experience preferred.
  • Maternal-Child position requires experience working with neonates, children and families.
