Job Description

This is a unique opportunity to make a real difference in the lives of our residents. As a Wellness Nurse, you will be responsible for conducting monthly wellness visits, monitoring resident health status, and providing clinical support to our care team. You will also assist with resident evaluations, data gathering, and documentation, as well as maintain effective communication with residents' families and healthcare providers.

Responsibilities
  • Identify ongoing needs and services of residents to promote high-quality resident services.
  • Ensure proper follow-through and participate in evaluation and documentation for residents with changes in health care services and status.
  • Assist Resident Care Director (RCD) in conducting resident evaluations and gather information at time of move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Conduct monthly wellness visits for all residents.
  • Monitor and follow up with residents who have experienced any falls, accidents, or incidents; document and report as required and directed.
  • Maintain accurate records of resident wellness files.
  • Maintain effective communication with residents' families and/or responsible parties to ensure medical needs are met.
  • Contact residents' attending physicians when necessary and/or upon family request.
Requirements
  • Graduate of an approved college/school of nursing.
  • Holds a current state license as a Professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Demonstrates knowledge of good assessment skills.
  • Able to handle multiple priorities.
  • Possesses excellent written and verbal communication skills.
  • Competent in organizational and time management skills.
  • Demonstrates good judgment, problem-solving, and decision-making skills.
Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as comprehensive benefits, including medical, dental, vision, and disability insurance.
