Key Responsibilities
  • Provide direct care to residents, including administering medications, performing wound care, and assisting with daily living activities.
  • Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care delivery and effective communication.
  • Monitor and report changes in residents' conditions, maintaining accurate and detailed records.
  • Participate in ongoing education and training to enhance skills and knowledge, staying up-to-date on industry best practices.
Requirements
  • Current LPN licensure in applicable state.
  • Minimum six months to one year of related experience preferred.
  • Valid driver's license and reliable transportation.
  • CPR certification and ability to meet health requirements, including TB testing and immunizations.
  • Pass background check and random drug tests.
