Job Title: Mental Health Case Manager

The Independence Center is seeking a highly motivated and compassionate Mental Health Case Manager to join our team. As a Mental Health Case Manager, you will be responsible for providing individualized support and services to individuals with serious and chronic mental illness.

Job Summary:

The Mental Health Case Manager will work closely with members to assess their needs, develop and implement individualized treatment plans, and provide ongoing support and services to promote their mental and physical well-being.

Responsibilities:
  • Conduct mental health assessments and develop individualized treatment plans
  • Provide case management services, including linking members to community resources and providing referrals to meet their needs
  • Assist members in obtaining public entitlements, housing, and community resources
  • Provide community support services and assist members in achieving and maintaining rehabilitative, resiliency, and recovery goals
  • Coordinate and transport members to appointments with mental and physical health medical providers
  • Conduct non-medical assessments and provide referrals for appropriate services
  • Provide ongoing outreach and support to members who have been absent from the center's programs
Requirements:
  • Bachelor's degree in social work, psychology, sociology, or related discipline
  • Minimum one year of experience working with individuals with serious and chronic mental illness
  • Strong interpersonal, mediation, and conflict resolution skills
  • Able to demonstrate sound judgment and demeanor in dealing with safety issues, emergencies, and high conflict situations
  • Valid driver's license and proof of insurability
Preferred Qualifications:
  • QMHP (Qualified Mental Health Professional) or LHPA
