Job Title: Registered Nurse - Case Management

VNS Health is seeking a skilled Registered Nurse to join our team as a Case Manager. As a key member of our healthcare team, you will play a vital role in providing high-quality, patient-centered care to our clients.

Job Summary:

The Registered Nurse - Case Manager will be responsible for coordinating and overseeing the delivery of care to our clients, ensuring that their needs are met and that they receive the best possible outcomes. This will involve working closely with our interdisciplinary team, including physicians, therapists, and other healthcare professionals, to develop and implement individualized care plans.

Key Responsibilities:
  • Coordinate and oversee the delivery of care to clients, ensuring that their needs are met and that they receive the best possible outcomes.
  • Develop and implement individualized care plans in collaboration with our interdisciplinary team.
  • Provide education and support to clients and their families to promote health and well-being.
  • Collaborate with our team to identify and address any barriers to care, such as social determinants of health.
  • Ensure that all care is delivered in accordance with our organization's policies and procedures, as well as relevant laws and regulations.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in NYS.
  • Certified Case Manager - Commission for Case Manager Certification within 1 year of employment.
  • Minimum two years of experience in a healthcare-related field, with a focus on case management or care coordination.
  • Proficiency in Microsoft Office applications and analytical skills.
