Job Summary: We are seeking a highly motivated and experienced Housing Stabilization Case Manager to join our team at Region Ten Community Services Board. The successful candidate will be responsible for providing case management services to chronically homeless adults who are participants in the Permanent Supportive Housing (PSH) rental assistance program.

Key Responsibilities:

  • Provide case management services to participants in the PSH program, including maintaining and accessing safe and stable housing, accessing medical and psychiatric care and services, and enhancing independent living skills.
  • Conduct service needs assessments, advocacy, consultation, and education to support participants in achieving their goals.
  • Collaborate with community partners to ensure a comprehensive and coordinated approach to addressing the needs of participants.
  • Develop and implement individualized service plans to support participants in achieving their goals.

Requirements:

  • Bachelor's degree in a related field and QMHP certification are preferred.
  • Valid Virginia Driver's License and acceptable driving record.
  • Ability to work independently with initiative and judgment, based on application of standard practices and with guidance from the supervisor.

Work Environment: This is a grant-funded position, and the length of the position is dependent on the life of the grant. The successful candidate will be expected to function in a fast-paced environment with multiple priorities and deadlines.
