Job Title: RN Care Manager

We are seeking a skilled RN Care Manager to join our team at Marysville Care Center.

About the Role:

The successful candidate will be responsible for coordinating patient care and services, promoting quality care using a collaborative process that coordinates, monitors, and evaluates services according to the needs of patients.

This role involves working together with the patient and family, care team, payers, and external entities to promote a safe transition from the facility to the next provider or care setting in accordance with all applicable laws, regulations, and Life Care standards.

Responsibilities:
  • Coordinate patient care and services
  • Promote quality care using a collaborative process
  • Work with patients, families, care teams, payers, and external entities
  • Maintain accurate records and reports
  • Communicate effectively with patients, families, and healthcare professionals
Requirements:
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Current licensure/registration in the applicable state
  • One year of clinical experience in post-acute care settings preferred
  • Prior case management, utilization review, and discharge planning experience preferred
