About the Role

We are seeking a highly motivated and compassionate Family Centered Treatment Specialist to join our team at Health Connect America. As a Family Centered Treatment Specialist, you will work closely with at-risk youth and their families, utilizing the Family Centered Treatment (FCT) model to provide comprehensive case management and support.

Key Responsibilities
  • Conduct individual and family sessions within the community to provide mental health support and guidance.
  • Work collaboratively with interdisciplinary teams to ensure coordinated care and support for clients.
  • Provide comprehensive case management to support the mental health and overall well-being of clients.
  • Develop and implement treatment plans that address the unique needs of each client.
Requirements
  • Minimum of a Bachelor's Degree in human services or a related field.
  • Two years of experience working with youth or families.
  • Current Family Centered Treatment Certification is preferred.
  • Additional state-specific requirements include: QMHP or ability to register as a QMHP in NC and VA.
