Job Summary:The Hospice LVN Continuous Care Evening Position is responsible for providing professional nursing care to patients in their homes or facilities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:- Provide patient care under the supervision of an RN
- Assess patient needs and develop individualized care plans
- Implement and document teaching plans based on patient and family needs
- Document all care provided to patients, including telephone interactions with other staff members
- Participate in the development and implementation of patient care plans
- Collaborate with the RN to ensure timely and effective patient care
- Vocational Nurse Training
- Minimum 12 months of experience as an LVN, with preferred experience in acute care, home health, or hospice
- Current California LVN license
- Familiarity with computerized bedside patient documentation
- Ability to function as part of a team and take direction from other team members
- Phlebotomy certification a plus
- Bi-lingual (Spanish) a plus
