Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Associate's degree required, BSN preferred.
- 2-3 years of hospital nursing experience required, with preference given to those with acute care (ICU, Med-Surg, Tele, etc.) experience.
- Case management experience a plus.
- Certification in case management or utilization review preferred.
- InterQual experience preferred.
HCA Florida Mercy Hospital is a 488-bed facility, accredited by the Joint Commission, and offering a wide range of medical specialties, including emergency care, heart and vascular care, and maternity and neonatal care. We are a Level II NICU hospital and CARF accredited for inpatient rehabilitation. Our facility offers a Comprehensive Community Cancer Program, accredited by the American College of Surgeons Commission on Cancer. We are affiliated with HCA Florida Healthcare Division and sponsored by the Sisters of St. Joseph of St. Augustine, Florida.
