Job Title: Skilled Nursing Professional

Aveanna Healthcare is seeking a skilled nursing professional to join our team as a Registered Nurse (RN) in Skilled Care. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities:
  • Deliver High-Quality Patient Care: Provide skilled nursing care to patients in their homes, adhering to their individualized care plans and physician orders.
  • Assess and Document Patient Needs: Continuously observe and assess patient condition and care needs, documenting all activities, assessments, and nursing actions in a timely manner.
  • Develop and Implement Care Plans: Develop, implement, and update nursing care plans to ensure patients receive comprehensive and coordinated care.
  • Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
  • Maintain Accurate Records: Maintain accurate and up-to-date patient records, including progress notes, medication lists, and care plans.
Requirements:
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • License: Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Certification: Current CPR certification.
  • Experience: One year of experience in a clinical care setting, with a focus on skilled nursing care.
Preferred Qualifications:
  • Home Health Experience: Previous experience in home health care, with a focus on skilled nursing care.
  • Specialized Skills: Experience with vital assessment, plan of care development, and telephone triage.
Physical and Environmental Demands:
  • Physical Demands: Must be able to lift 50 pounds, walk, stand, bend, kneel, reach, and twist.
  • Environmental Demands: Must be able to function in a wide variety of environments, including exposure to allergens and infectious diseases.
