The ideal candidate will possess excellent communication skills, a good understanding of technology, and community engagement skills. With access to personal, reliable transportation, you will be able to work effectively with a caseload of up to 25 clients, providing face-to-face time, collateral contacts, travel time, paperwork, planning, knowledge building, and resource building. In this role, you will have the flexibility to work independently while collaborating with internal staff and community partners to ensure seamless service delivery.

As a Targeted Case Manager at Emerald Therapy Center, you will be responsible for:

  • Managing a caseload of up to 25 clients, including face-to-face time, collateral contacts, travel time, paperwork, planning, knowledge building, and resource building.
  • Developing comprehensive targeted case management assessments and periodic reassessments of assessments on all clients entering your caseload.
  • Providing ongoing case management that is supportive and in accordance with the client's needs, objectives, and goals identified in the treatment plan.
  • Making necessary revisions to the treatment plans when new issues arise to ensure the client's needs are being addressed.

To succeed in this role, you must have a Bachelor's degree in Social Work, Sociology, Psychology, or other related human services fields. You should also be able to maintain certification requirements for TCM pursuant to Kentucky Board regulations.
