CCM/MDS Director

Job Summary: We are seeking an experienced MDS Director to lead our clinical operations team at Nursecare of Buckhead. The successful candidate will be responsible for coordinating and facilitating the timely and accurate completion of both the RAI and Care Management process from admission to discharge in accordance with company policies, state and federal guidelines, and all other relevant entities.
Key Responsibilities:
- Coordinate and manage daily care management meetings, including review of resident care and setting of the Assessment Reference Date.
- Enforce accurate and timely resident assessments according to state and federal regulations.
- Designate responsibility for completing sections of the assessment to the interdisciplinary team.
- Complete accurate coding of the Minimum Data Set (MDS) with information obtained via medical record review, observation, and interview with facility staff, residents, and family members.
- Maintain the tracking system of MDS schedules (time frames and due dates).
- Maintain frequent and accurate data entry of resident information into appropriate computerized MDS programs.
- Coordinate care plan conferences with the interdisciplinary team, residents, and families.
- Obtain, review, and maintain all state and federal reports, making timely corrections as necessary.
- Coordinate education related to the RAI/Care Management process for the facility.
- Coordinate the electronic submission of required documentation to the state database and other entities per company policy and state and federal regulations.
- Complete required forms and documents in accordance with company policy and state and/or federal regulations.
- Attend interdisciplinary team meetings and other meetings to gather information, communicate changes, and maintain and update records.
- Contact the Regional Clinical Reimbursement Consultant as necessary.
- Continually update knowledge base related to data entry and computer technology.
- Participate in quality assurance activities.
- Perform other duties as assigned.
Requirements:
- RN preferred or LPN/LVN with approval from the facility Administrator and appropriate District/Regional staff.
- Must be in good standing with the Board of Nursing.
- Knowledge of Medicare process preferred.
- Current state nurse license.
- CEUs as required by the state.
Working Conditions:
The employee will work in a normal office environment and may be required to sit, bend, stoop, use a keyboard, see, talk, and hear. The employee may occasionally lift medium to heavy objects, including the movement of residents. The employee may be exposed to blood or other bodily fluids, fumes, or airborne particles and toxic or caustic chemicals.
