Key Responsibilities- Conduct thorough physical therapy evaluations and develop personalized treatment plans to meet patients' unique needs.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
- Provide one-on-one care to patients, focusing on improving their functional abilities and overall quality of life.
- Stay up-to-date with the latest rehabilitation techniques and best practices to deliver exceptional patient outcomes.
- Supervise and mentor Physical Therapist Assistants and Physical Therapy Aides to ensure high-quality patient care.
- Graduate of a Physical Therapist Education program approved by CAPTE.
- Current Physical Therapist licensure in the state and American Heart Association CPR certification.
- At least two years of experience in adult physical therapy, with a preference for home health experience.
- Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
