Physical Therapist Opportunity

Revival Home Health & Hospice is seeking a dedicated Physical Therapist to join our team in Annandale, VA. Responsibilities
  • Perform physical therapy evaluations and treatments based on physician orders and referrals.
  • Complete comprehensive evaluations using appropriate evaluation and assessment tools and procedures.
  • Review patient records for current diagnosis, past medical history, precautions, and contraindications.
  • Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function.
  • Regularly re-assess the effectiveness of treatment plans, progress of patients toward goals, and the need for modifying goals or treatment.
  • Communicate patient needs and progress to the treatment team, physician, person receiving services, family members, as applicable.
  • Consult with medical providers regarding patient evaluation and treatment, progress of patients, and discharge from treatment.
  • Recommend and assist in securing special adaptive equipment for patients to improve function and incorporate the use of adaptive equipment to enhance the rehabilitation program.
  • Maintain and submit evaluation, treatment, and billing documentation in the format and timelines required by the company, facility, funding source, local, state, and federal regulations.
  • Adhere to standards of the company's Code of Business Conduct, policies, Health Insurance Portability Accountability Act, and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
  • Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act.
  • Attend, participate in, and/or conduct internal staff development programs and meetings; obtain continuing education as required by company policy and regulations; maintain and enhance clinical practice skills.


Qualifications
  • Graduate of a Physical Therapist Education program approved by CAPTE.
  • Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire.
  • Two (2) years of experience in adult physical therapy, home health experience preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills; physical therapy skills per competency checklist.
