Job Summary

The Certified Wound Ostomy Nurse coordinates and provides care in the home care setting to patients with ostomies, draining wounds, fistulas, pressure ulcers, acute and/or chronic wounds, and incontinence. This role requires a high level of expertise in wound care and ostomy management, as well as excellent communication and interpersonal skills.

Key Responsibilities

  • Provide hands-on care to patients with ostomies, draining wounds, fistulas, pressure ulcers, acute and/or chronic wounds, and incontinence.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare team members to ensure comprehensive care and coordination of services.
  • Provide education and support to patients and their caregivers on wound care and ostomy management.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.

Requirements

  • Bachelor's Degree in Nursing
  • California Registered Nurse License upon hire
  • National Provider BLS - American Heart Association within 30 days of hire
  • 1 year Nursing experience

Preferred Qualifications

  • 1 year CWOCN (ET) experience
  • 2 years RN experience
  • 1 year Home Health experience
