Case Manager for Quality Patient Care

We are seeking a skilled Case Manager to join our team at Mass General Brigham, where you will play a vital role in ensuring high-quality patient care. As a Case Manager, you will be responsible for coordinating and managing patient care to achieve desired clinical and financial outcomes.
Job SummaryThis is an exciting opportunity to work with a dynamic team of healthcare professionals who share your passion for delivering exceptional patient care. You will work collaboratively with patients, families, and interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Coordinates and implements patient care plans to ensure timely and effective transitions between care settings.
- Collaborates with physicians, nurses, and other healthcare professionals to assess patient needs and develop comprehensive care plans.
- Communicates effectively with patients, families, and caregivers to ensure understanding and engagement in the care planning process.
- Maintains accurate and up-to-date records of patient information, including medical history, treatment plans, and progress notes.
- Works closely with insurance providers and payers to ensure seamless coordination of care and minimize delays in treatment.
- Identifies opportunities for improvement in care processes and participates in quality improvement initiatives.
- Bachelor's degree in Nursing or related field required; Master's degree preferred.
- Minimum 1 year of experience in case management or a related field, preferably in a hospital or healthcare setting.
- Excellent communication, organizational, and interpersonal skills required.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Familiarity with electronic health records and other healthcare software systems.
The estimated annual salary for this position is $85,000 - $110,000, depending on experience. In addition to competitive compensation, we offer a comprehensive benefits package, including medical, dental, vision, and life insurance, as well as employee assistance programs.
