About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Nurse Case Manager. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and coordinating services to meet the unique needs of our patients.

Responsibilities
  • Conduct thorough assessments of patients' medical and social needs, developing and implementing individualized care plans to ensure optimal outcomes.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families, empowering them to take an active role in their care and recovery.
  • Manage and coordinate the delivery of home health services, including medication management, wound care, and other interventions as needed.
  • Participate in regular case conferences and team meetings to review patient progress and adjust care plans as necessary.
Requirements
  • Current licensure as a Registered Nurse in the state of employment.
  • A minimum of one year of clinical experience, preferably in home health or a related field.
  • Strong knowledge of current nursing practices, including OASIS collection and medication management.
  • A valid state driver's license and automobile liability insurance.
