Job SummaryWe are seeking a highly skilled Registered Nurse RN Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating patient care, developing and implementing individualized care plans, and collaborating with healthcare professionals to ensure seamless care transitions.
Key Responsibilities- Conduct comprehensive patient assessments to identify care needs and develop individualized care plans
- Coordinate with healthcare professionals to ensure timely and effective care delivery
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
- Required Certifications and Licensures:
- Licensed to practice as a registered nurse in the state of agency operation
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Competitive salary range: $84,000 - $90,000 per year
