Join Our Team as a Registered Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at AccentCare. As a key member of our home health team, you will play a vital role in delivering high-quality patient care and improving the lives of our patients and their families.

About the Role

This is a full-time position that requires 40 hours of work per week, with a shift of 8 hours. You will be responsible for providing case management services to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address patients' needs and goals
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions
  • Provide education and support to patients and families to promote self-care and independence
  • Monitor patients' progress and adjust care plans as needed
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing and current licensure as a registered nurse in the state of agency operation
  • At least one year of experience as a registered nurse, preferably in home health or case management
  • Valid CPR certification and a reliable vehicle for travel
