About the Role

Cleveland Clinic is seeking a highly skilled Clinical Pharmacist to join our team at South Pointe Hospital. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications for our patients.

Key Responsibilities
  • Collaborate with healthcare teams to develop and implement medication therapy plans that meet the unique needs of each patient.
  • Conduct thorough medication reviews to identify potential safety risks and develop strategies to mitigate them.
  • Provide education and training to patients, families, and healthcare staff on medication use and safety.
  • Stay up-to-date on the latest research and guidelines in pharmaceutical care and apply this knowledge to improve patient outcomes.
  • Work closely with our pharmacy team to ensure seamless medication delivery and management.
Requirements
  • PharmD or Bachelor's degree in Pharmacy from an accredited institution.
  • Current licensure as a Registered Pharmacist (RPh) or eligibility for reciprocity in the state of Ohio within 90 days of hire.
  • Board certification in a relevant specialty is preferred.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy-approved course in immunization administration are required for individuals who administer immunizations.
  • Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the AHA is required for individuals who respond to adult or pediatric cardiopulmonary arrest events.
Preferred Qualifications
  • Completion of a post-graduate year 1 residency (PGY1) or fellowship in a relevant area.
  • Post-graduate year 2 residency (PGY2) or relevant experience in a specialized pharmacy practice area, such as critical care or oncology.
