About the Role

We are seeking a highly skilled Palliative Care Nurse Specialist to join our team at Illumia. As a key member of our Advanced Illness Management (AIM) team, you will play a critical role in providing exceptional patient care and support to individuals and their families facing advanced illnesses.

Key Responsibilities
  • Provide comprehensive palliative care services to patients and families, including assessment, intervention, and teaching to maximize patient comfort and quality of life.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to develop and implement individualized care plans.
  • Respond to crisis calls from patients and families after hours, providing timely and effective support and guidance.
  • Conduct initial nursing evaluations to determine eligibility for palliative services, and re-assess patient needs as necessary.
  • Document patient progress, problems, goals, interventions, and responses to care in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
Requirements
  • Current RN license to practice in the state.
  • 2 years of prior skilled Registered Nurse experience, preferably in palliative care, oncology, hospice, or geriatric care.
  • Demonstrated ability to provide quality patient care and strong compassionate phone skills.
  • Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel, as well as electronic medical record documentation.
