MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Twilight Home. As a key member of our healthcare team, the successful candidate will be responsible for ensuring the accuracy and completeness of resident assessments and care plans.

Key Responsibilities:

• Schedule and coordinate resident assessments, ensuring timely completion and accuracy of MDS documentation
• Monitor and maintain accurate and up-to-date resident care plans, ensuring compliance with federal and state regulations
• Collaborate with nursing and other departments to ensure seamless communication and coordination of care
• Attend regular Medicaid meetings to discuss resident care and updates
• Perform care plan updates and transmit MDS data in a timely manner

Requirements:

• Licensed Nurse in good standing
• Geriatric nursing background and experience
• Excellent organizational and communication skills
• Ability to work independently and as part of a team
• Working knowledge of nursing home regulations and industry standards
