We are seeking a skilled MDS Coordinator to support our team at Twilight Home. The ideal candidate will have a strong background in geriatric nursing and be proficient in software.

The MDS Coordinator will be responsible for scheduling residents for assessments, ensuring compliance with federal and state regulations, and maintaining accurate documentation.

Key responsibilities include:

  • Scheduling residents for assessments and communicating with nursing and other departments
  • Ensuring compliance with federal and state regulations regarding the comprehensive assessment (MDS)
  • Maintaining proficiency in software and coordinating with ITTM meetings
  • Providing the Quality Indicators Report to DON, Administrator, and pertinent staff
  • Performing transmissions of the MDS on a timely basis

The successful candidate will have a licensed nurse background, geriatric nursing experience, and excellent communication and organizational skills.
